As a Care Coordinator - RN you will provide a deliberate patient and family centered partnership with the healthcare team by developing and supporting a coordinated plan of care between visits (Inpatient and Outpatient) to promote a smooth transition for patients and families across the healthcare continuum.

Our Electrophysiology department is dedicated in providing cutting-edge care and innovative treatments for patients with heart rhythm disorders. Our services include catheter ablation, implantation of pacemakers and defibrillators, and cardiac resynchronization therapy. The Care Coordinator - RN works to maximize the patient's health outcomes and experience by identifying patient and family needs, completes pre-visit planning, supports clinic visits, and provides closure to the clinic visits. The Care Coordinator - RN utilizes coaching to help ensure patient and family participation in treatment plans and provides exceptional customer service to internal and external customers (providers, clinic staff, patients, and families).
