Summary: The Stroke Coordinator provides oversight and coordination of all elements of the Stroke Program, in collaboration with the leadership and the Medical Director. Responsibilities include the planning, organizing, and evaluation of patient care within the program as well as compliance with regulatory bodies and national guidelines for stroke management.
Other information:
Minimum 2 years clinical experience required. Clinical expertise caring for stroke patient populations required.
Must document a minimum of 8 hours of Stroke specific education, and an additional 8 hours of other cardiovascular related education within 90 days and annually-Required.
CPR and ACLS required.
Familiarity with (GWTG) Get With The Guidelines - Stroke American Heart Association Preferred.
Bachelor's degree in Nursing (BSN) - Required, or in pursuit of Masters Degree Preferred

Responsibilities: In collaboration with nursing and other team members, monitors ongoing assessment, planning, implementation and evaluation of outcomes of stroke and heart failure patients. Provides ongoing clinical education relating to the medical management of primarily stroke patients. Works closely with medical staff and interdisciplinary clinicians to proactively identify clinical needs of the CV service line and develop a plan to meet those needs. Identifies and adapts criteria of evidence based medicine to ensure appropriate care. Daily monitors appropriateness of care based on evidence based medicine. Recommends clinical pathways and current best practices to obtain better clinical outcomes. Performance Improvement: Through evaluation of data and outcomes, identifies opportunities for improvement, works with team to develop a plan for improvement, implements the improvements and evaluates success followed by modifications to plan if necessary by monitoring data. Clinical resource for care and education of the stroke patient and/or caregiver. Development of patient teaching programs directed at stroke/TIA patients. Decision Making: Ability to make sound decisions regarding case management of medical CV patients. Remains current in knowledge of TJC disease specific standards to assure compliance as a Primary Stroke Center. Performs statistical analysis of quality data for trending.
