The VA Northeast Ohio Healthcare System is recruiting for a Physical Therapist for the Wade Park VA Clinic. The Physical Therapist will function within the Home Based Primary Care section in Community Outpatient Service. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: Citizenship: citizen of the United States. Non-citizens may be appointed when it is not possible to recruit qualified citizens. Education: graduate of degree program in Physical therapy from a college or university accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE) with a Baccalaureate degree or higher in Physical Therapy. Licensure: current, full, active and unrestricted licenses to practice Physical Therapy in a state, territory, or commonwealth of the United States, or in the District of Columbia. English language proficiency: spoken and written English language proficiency in accordance with VA Handbook 5005, Part II, Chapter 3, Section A, Paragraph 3j. The qualified individual will meet and maintain the qualification requirements for the position, assignment and grade as outlined in the Physical Therapist Qualification Standard, VA Handbook 5005/24, Part II, Appendix G12. The qualified individual will maintain a non- restricted driver's license. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-12: In addition to the basic requirements, candidates must possess one year of experience equivalent to the GS-11 grade level and hold a valid unrestricted state license. Examples of experience equivalent to GS-11 are as follows: skill in performing examinations and evaluations of individuals who have or may develop impairments, activity limitations, and participation restrictions related to conditions of the musculoskeletal, neuromuscular, cardiovascular, pulmonary, and/or integumentary systems while considering the effects attributable to unique psychosocial and environmental factors; ability to interpret findings from examination and evaluation, select appropriate test and measures, and integrate findings into the physical therapy plan of care for the full range of patient populations; and ability to determine physical therapy prognosis by incorporating examination findings with the patient's preferences in order to set clinically appropriate treatment goals, optimize outcomes and maximize functional independence. Assesses all patients who are consulted to HBPC for dysfunction using appropriate tests/measures, evaluates findings, and develops a plan of care with functional and measureable goals within appropriate timeframe that are based on assessment findings Demonstrates the competency to treat and provide education to young adults across the lifespan to include geriatric patients and refer to other medical professionals as medically indicated. Establishes and maintains contact with other health care providers involved in patient care via written, telephone and personal communication on a regular basis at the Cleveland VA Medical Center including CBOCs. Functions autonomously in carrying out patient education, specialized treatments, and applying physical therapy procedures for which competency has been established and in accordance with laws or regulations and APTA documents. Completes all required patient care computerized documentation in a timely fashion, including initial evaluation, daily treatment, interdisciplinary care plans, progress notes, telephone contacts, consults, re-assessment and discharge notes in accordance with VA policy. Consults with patients and their families to educate them on their condition, instructs them in home exercise programs, use of medical equipment, home and community safety. Maintains professional licensure and completes all required annual continuing education for license renewal. Participates in the development and implementation of quality assurance monitors and quality improvement procedures in the service line and through participation in hospital or CBOC committees. Pro-actively manages patient scheduling and collaborates with colleagues to ensure all consults are completed in a timely fashion according to medical center policy. Maintains VISTA security at all times.
