Job Summary:* Has knowledge of community health concepts, principles of ambulatory clinical nursing care, case management and care coordination. Assumes responsibility and accountability for facilitating, communicating and collaborating with the healthcare team and the patient/ family, utilizes best practice to maximize patient engagement and patient self management efforts. * Promotes the optimal health, well-being and safety of the patient through use of the nursing process and in accordance with patient care standards, guidelines and the State Nurse Practice Act. Demonstrate personal accountability for relationship-based care, organizational mission and core valuesEssential Funtions:* Assess patient's physical, psychosocial, cultural and spiritual needs through observation, interview, review of medical records & clinical data. * Evaluates patient's response and intervenes to ensure optimal patient outcome. Assess patient's ability to participate to the fullest extent by determining presence/ absence of barriers to active participation in the individualized care plan.* Develop and implement patient plan of care and observe outcomes in accordance with nursing standards and in collaboration with the interdisciplinary team.* Demonstrate competency in a variety of therapeutic/diagnostic interventions including but not limited to IV's, incision/wound care, medication administration, catheterization and specimen collections. * Promote, advocate and collaborate to protect the health and safety and rights of each patient. Anticipates the needs of the patient to maximize use of physician visits.* Provide patient/family education through assessment of health literacy, barriers to learning readiness, needs, and ability. Provide teaching and evaluate effectiveness of teaching.Qualified external applicants with direct clinical experience are eligible for up to a $20,000 sign on bonus! The bonus will be taxed per IRS regulations. Additional relocation bonus may apply based on geographical location. Must be hired into a full-time or part-time position; PRN positions are not eligible. Some exclusions may apply. $400/month Summit County housing stipend available for full time residents of Summit, Park, Eagle, Grand, Clear Creek or Lake Counties.
