Experience in a skilled long-term care setting is preferred. The selected physician will address the goals of care and quality of life of patients facing serious illness while incorporating the patient's physical, psychological, and spiritual well-being. The incumbent is to be a physician that has completed an appropriate residency or fellowship, board certified or board eligible in Geriatric Medicine, Internal Medicine, or Family Medicine with substantial experience in geriatrics and long-term care. Certification in Basic Cardiac Life Support is required. Advanced Cardiac Life Support certification is recommended. The physician is primarily assigned attending responsibilities in the CLC. The physician is responsible for providing timely admission H&P, discharge summaries, acute visits, and routine follow up notes. The physician's job performance includes the assessment of acute and chronic conditions, diagnosis & management plan, referrals as deemed necessary, interpretation of tests, including lab results, pathology, and diagnostic imaging, inpatient or outpatient consultations, supervision of residents and rotating medical trainees, and evaluating referrals for inpatient admission or consultation. Physician role will include leading and participation in family conferences. Must be comfortable and skilled in initiating and facilitating goals of care conversations, including end of life discussions involving shared decision making with the patients, their loved ones and/or surrogate decision makers. The incumbent also participates in and directs activities pertaining to quality improvement, strategic planning, Long Term Care Institute (LTCI) and The Joint Commission accreditation process and is an active member of an interdisciplinary team.
