This Nursing Assistant for the Community Living Center is located at the Wilkes-Barre VA Medical Center in Nursing Service and reports to the Nurse Manager of the assigned unit. The Nursing Assistant selected for this position will need the basic knowledge and skills necessary for a concentrated effort and is somewhat independent in carrying out standardized procedures of limited complexity. The incumbent will be closely supervised when providing safe, effective care to patients/residents. Actives will include bathing, personal hygiene, assisting with feeding, taking-recording-reporting vital signs accurately, assisting ambulation and other duties or care as directed by RNs, LPNs or providers. The Registered nurse, charge nurse and/or supervisor make daily assignments at the beginning of the shift. After initial assignment the employee sets priorities and deadlines for patient care. The employee uses initiative in carrying our recurring tasks independently with our specific instructions but refers problems or deviations not covered by instructions to the professional nurse. Upon completion of the tour, important information is reported to the oncoming shift to assure continuity and quality patient care is upheld. The Nursing Assistant will be required to observe patients/residents who may be confused or have other cognitive issues providing safe environment and reports to nurse when condition changes. Incumbent will initiate emergency action in life threatening situations such as cardiac and /or respiratory arrest, massive bleeding, shock, or assaultive behavior and alerts licensed personal immediately. Duties include but not limited to the following: provide a range of patient/resident care and ability to learn how to contribute to the completion of the nurse care plan and recovery of patients. appropriately observe patient's/resident's physical or emotional status and refer deviation and/or problems to supervisor in a timely manner. This includes safety & 1:1 watch observations following PMDB competency. properly use equipment, materials and supplies in simple diagnostic and treatment procedures such as bladder scan, continuous passive motion device, specimen collection, etc. communicate effectively with patients/residents, their families and other health professionals.
