This Registered Nurse Cardiology Navigator is located in the Medical Service (Cardiology Service) at the West Roxbury campus location. This position is a full-time position 40 hours per week. This is a Registered Nurse (RN) who provides clinical support by managing the transfer and bookings of Veteran patients requiring Cardiac Care in VISN 1. The incumbent will act as a liaison between VA Boston Healthcare System (VABHS), the VISN 1 Clinical Resource Hub, other VA facilities, and private sector institutions to facilitate and improve access to VA Cardiac Care. The RN Cardiology Navigator duties include, but are not limited to: Responsible for collaborating and effectively communicating with providers outside of VABHS to schedule patients for cardiology procedures and facilitate the smooth transfer of inpatients from other institutions to the Cardiology Service. Responsible for direct communication with the Veteran outpatients to schedule procedure(s), provide information and teaching, including review of medications, and arrange transportation when necessary. Ensure that all imaging studies, blood tests, and all other pertinent data are obtained prior to procedure, either at VABHS or at the referring facility. Work closely with the Nurse Practitioners and Physicians to ensure appropriate triage and care of referrals. Responsible for confirming results of studies and for communicating any follow-up information to the referring provider. Ensure that key clinical information from VABHS (e.g., imaging, lab studies, medication changes, clinical plans, etc.) is documented into the patient-side electronic medical record to ensure that referring providers are aware of clinical plans. Responsible for maintaining an electronic spreadsheet of all transfers and outpatient bookings. Provides care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, health counseling. Responsible for the provision of care supportive to or restorative of life and well-being. Executes medical regimens including administering medications and treatments prescribed by a licensed or otherwise legally authorized provider. The salary is based on the grade and step approved for the selected candidate. The Professional Standards Board (a peer-review group) will recommend the grade and salary for new appointments based on the candidate's education and experience. For internal selectees being promoted, the board will determine grade and HR will determine step in accordance with policy.
