The Northeast Ohio VA Health Care System is recruiting for a Licensed Practical Nurse, in the Pulmonary Outpatient Clinic within the Medicine Service at the Cleveland VA Medical Center. Assists with the planning, implementation, and evaluation of patients' care with all interdisciplinary teams. Functions in complex healthcare activities with minimal supervision and guidance. Demonstrates the application of the nursing process to quickly identify actual or potential, health, safety and educational needs and directs cases to appropriate providers or resources. Oversees coordination of care for the assigned patient population, providing oversight to administrative staff and coordination of multi-leveled Veteran care needs. Supports the RN or MD/DO in monitoring the practice of unlicensed nursing or non-medical staff. Promptly reports any deviations from normal practice patterns to the RN or MD/DO. Possesses the ability to assist nursing (RNs) and medical staff (MD/DO/PAs/ NPs) to appropriately prioritize overall Veteran care needs and make adjustment to the plan to deliver care. Demonstrates the ability to make astute observations of subtle, less obvious changes in condition and can accurately document care delivered in acute/urgent/stat conditions and take the necessary steps to ensure immediate attention to critical events. Completes comprehensive nursing documentation according to standards. Educates patients about their general care, side effects and recovery process. Utilizes advanced communication techniques to assist individuals/groups/family members in taking an active role in their care under very general supervision. Readily identifies patient complications and acts to resolve issues under very general supervision. Applies critical thinking skills to solve clinical and administrative issues. Exhibits skill, creativity, and initiative in contributing to the improvement of overall care delivery and the Veteran/family satisfaction levels through active participation in committees/tasks forces, research projects, or in developing educational materials/media. Actively promotes and effectively incorporates established customer service standards in all aspect of their practice. Able to proactively assist staff to identify Veteran medications/treatment needs, and accurately relay those needs to necessary/pertinent team members involved in the Veteran's care. Demonstrates a knowledge of specialty care nursing trends and evidence-based care in the assigned department. Work of considerable difficulty and responsibility is performed under general supervision. Individuals in these positions are to demonstrate independent technical judgments and possess and apply a comprehensive working knowledge of complex practices and procedures successfully and consistently. Demonstrates a knowledge of PCCA nursing trends and evidence-based care in the assigned department. Demonstrates active collaboration and communication with disciplines within the team. Incumbent follows and assists with implementing accountability measures. Identifies and communicates problem areas between disciplines to improve outcomes. Readily identifies ethical dilemmas and appropriately consults h/her supervisor/chief/ MD or service or seeks ethics consultation. Provides indirect nursing care to patients with complex needs as related to consult management, screening of telephone calls, as well as assuming responsibility and accountability for nursing action. Demonstrates the ability to establish priorities, manage multiple tasks, and provide a professional response to other facilities and callers. Demonstrates active listening and excellent interviewing/screening skills during telephone calls to obtain pertinent information from the caller and utilizes computerized medical record as an additional information resource. Collaborates with other nursing staff/physicians/unit Directors and Service Chiefs regarding Veteran care and unit activities. Serves as a preceptor for new staff as needed and clinically appropriate. Actively seeks out educational opportunities to enhance nursing knowledge and skills and provide leadership in assisting other staff to improve and advance their nursing practice. Maintains and updates professional certifications as required by policy/procedure. Assures continual compliance with Joint Commission and other regulatory standards applicable to the unit's scope of practice by complying with guidelines, policies, and procedures.
