A recruitment/relocation incentive is not authorized for this position. Work Schedule: Monday through Friday 8:00 AM to 4:30 PM. Management reserves the right to schedule work to meet changing patient care needs. The Pulmonary Nurse Care Coordinator is responsible to provide direct nursing care to a culturally diverse veteran population of varying complexity in the Inpatient/Outpatient setting. The RN Care Coordinator will work closely with the Pulmonary Section Chief and other pulmonary providers on work which may include coordination/scheduling of pulmonary procedures, assisting with bedside procedures, and assisting with the Referral Coordination Initiative (RCI) within the Medicine Division to include: monitoring wait times and community care usage, improving referral timeliness and empowering Veterans with understanding the full range of their care options. The RN Care Coordinator will assist with the implementation of VA Video Connect, CVT, and telephone clinics. The RN Care Coordinator will assist in the development, implementation, and continued assessment of other pulmonary/sleep programs within the Pulmonary Section. Responsibilities: The RN Clinical Care Coordinator will collaborate with physician partners to perform nursing duties as assigned. The primary duties include but are not limited to: Assisting in triaging patient consults for clinics and procedures Assisting with the Referral Coordination Initiative within the Medicine Division to include monitoring wait times and community care usage, and productivity Assisting in the scheduling of bronchoscopies Identifying barriers to care including support and collaboration with internal/external healthcare providers Tracking patients with complex care needs and follow-up of patient no shows/canceled appointments Assist in the coordination of clinical efforts with other services within the medical center to include bronchoscopies and sleep studies Providing clinical assistance during non-emergent bedside procedures Developing/maintaining content expertise in clinical care and serving as a resource to other nursing staff within the facility Communicating with patients and collaborating with team members to ensure all issues related to patient care are addressed in a timely manner Monitoring and tracking available resources to ensure they are being used in a timely and cost-effective manner to obtain maximum value for the patient and facility All other duties and tasks to commensurate with the position as assigned by management Preferred qualifications: BSN preferred Pulmonary experience preferred Care coordination experience preferred Minimum of three (3) years clinical experience in an inpatient and/or clinic setting preferred Patient education experience preferred Effective interpersonal & communication skills a must High level of flexibility/adaptability needed to meet patient and medical team's needs Financial Disclosure Report: Not required
