This Licensed Vocational Nurse (LVN) position is located in the Primary Care Clinic at the Michael E. DeBakey VA Medical Center located in Texas City, TX. The incumbent offers services of outpatient management of chronic diseases and health maintenance programs, supportive therapy, consultative services, and follow-up care and resources to veterans. Current permanent VA employees and Federal employees from other federal agency should apply under CBST-11031747-21-LG The incumbent must possess the ability to work with the nursing staff, providers, and others and is able to communicate effectively with patients/family members, and members of the health care team. The incumbent is accountable and responsible for data collection, reporting accurate vital signs, height & weight, and performing clinical reminders at time of visit. The incumbent adheres to established policies, standards of care and standards of practice. Performs all Clinical Reminders during patient visit and notifies Provider of positive screens such as AUDIT-C alcohol screen and Tobacco screen. Responds timely to secure messaging (without escalation) and communication status to provider/nurse Performs post-discharge phone calls within 48-hours from discharge and documents in CPRS Takes vital signs, Height, and Weight with accuracy. For hypertension, records all 3 BP readings in CPRS and notifies provider/Charge Nurse. Performance of complex therapeutic procedures requires regular observation and guidance from the RN Care Manager/charge nurse. The LVN follows standard which cause the patient no harm by performing, reporting, and recording assigned nursing care including but not limited to: a) Vital signs including Pain as a 5th vital sign b) Dressing change, simple and complex c) Oxygen administration d) Urinary catheterization e) Pulmonary hygiene and drainage f) Administration of delegated procedures and treatments g) Administration of premixed IV piggybacks h) Administration of medications Additional duties as assigned Work Schedule: Monday thru Friday 8:00 am to 4:30 pm Financial Disclosure Report: Not required
